The above scheduled flights on scheduled airlines are open to those who are members at least six months prior to departure of the desired flight, and to
their spouse, dependent children and parents residing in the same household. NO OTHER PERSONS ARE ELIGIBLE.
Present airline regulations require the member to accompany the family member on the flight. Children 2.12 years of age may travel half-fare up to the
limit of ten on each flight. Children under two years carried by the parent must pay 10% of the fare. The ages of the children must be computed as of
the date of departure and NOT the date of the reservation. ALL BOOKINGS ARE ON A FIRST-COME FIRST-SERVED BASIS. Please make your reservations early.
No reservations can be made without a deposit. THE RESERVATION APPLICATION MUST BE RETURNED WITH A DEPOSIT OF $100.00 FOR EACH FULL FARE
BOOKING, AND $50.00 FOR EACH HALF-FARE. THE BALANCE OF THE PAYMENT IS DUE BY MARCH 1, 1969. Tickets on the flights will be sent about one month
prior to departure.
IN ORDER NOT TO JEOPARDIZE THE FLIGHTS SET UP, THERE WILL BE A CANCELLATION FEE Of $25.00 PER ADULT BOOKING ASSESSED AGAINST THOSE MEMBERS
WHO CANCEL UP TO 1 MONTH PRIOR TO DEPARTURE. BETWEEN 30-21 .DAYS OF DEPARTURE THE AIRLINES CHARGE A 10% CANCELLATION FEE, WITHIN 21 DAYS
OF DEPARTURE THE AIRLINES CHARGE A 25% CANCELLATION FEE.
IMMUNIZATION; Immunization against smallpox within the last three years is necessary. Proof of same is entered on an international health card (PHS-731) Shots
against Typhus, Tetanus, and Polio are advised but not required.
PASSPORTS: Passports are good for five years from issue date. Obtain your passports early from the U. S. Passport Office, either locally or from Washington, D. C.
ALIENS: Resident aliens must have their valid passports and green alien registration card, form 151, with them to permit re-entry into the United States.
You must also obtain a tax release form within 30 Days of Departure from your nearest Internal Revenue Service office. PLEASE 00 NOT PACK ANY DOCU.
MENTS IN YOUR LUGGAGE. CARRY THEM WITH YOU AT ALL TIMES. .
If necessary, we will ask members on a flight to supply approximately $50.00 each if any flight falls below 50 participants but not below 25. If a group
flight falls below 25 participants we reserve the right to offer alternative flights or to cancel the flight and refund In full minus a $10.00 administrative fee.
The airlines do not allow one-way or splitting of group reservations. In order to receive proper credit for your booking, ALL checks MUST be identified with the
name of the member traveling and agree with the application form. Please sign your checks.
