"Eye Disorders in Leukemia and Other Instances of Remoblastosia.11 Zagreb. Lijecnioki Vjesn 9 Vol 85, No 8. 1963, pp 893-894. Abstract: The author reviews recent world literatureq including one article in which 50 percent of a group of 76 oases of terminal leukemia was described as having changes in the eyes. Such changes were more common in acute leukemia than in chronic leukemia. Histological changer, consisted mainly of leukemic infiltrations and hemorrhages in various combinations and with varying severity. Vascular structures in the eye were the most commonly affected tissues. The author acknowledges that a review of changes in the eyes in oasea of leukemia and other instances of hemoblastovisp some of which are extremely rare, may appear without great practical value but that eye symptoms as the first manifestation of the leukemic process can eauso differential-diagnostio confusion be- cause of the early lack of systematic disturbances. These data serve as the chief basis of the author's conception of Igeneral inhibition' and of his hypothesis concerming the 'neurone-neuropill structure of the central nervous system in vertebrates. In the experiments,, normal animals, and lumbar and spinal preparations (of a cat, rabbit and frog) were used. Reflex reactions and inhibition were induced (1) by electrical stimulation of sensitive nerves, the dorsal roots of the lumbar and sacral segments; (2) chemically, by applying to the spinal cord a solution of acetylcholine (D2,000-50,000), nicotine, KC1; (3) mechanically by pressing the skin and touching the surface of the spinal cord with a brush. The movement of muscle groups (antagonist) of the knee or of whole extremities were registered myographically. Excitation: New physiological proofs confirm what is well4mown to histologists i.e. that definite dorsal root fibres are connected in the spinal cord with the motor neurones of the homolateral sido, both directly and throxigh tho intornunclal neurones and with the motor neuronott of the contrnlotoral side only- throtigh ow- internuncial neurones. On the basis of the author's own ph-ysiological observations and the histological data of Nemiloff, Ramon-y-Cajal, Dogiel, Lorente de No and others in this work an original scheme of the structure of the spinal cord- interneurone connections - is given. Inhibitions With any'sensory stimulation - even subliminal - of the akin receptors, sensory nervesp roots, and spinal cord, both (antagonistic) groups of muscles of the hind extremities are always inhibited; consequently inhibition in all cases is general. Inhibition can be shown also in the excited muscles, if, during the reflex twitches induced'by the stimulation of one root, the other root is stimulated by a faradic current, giving reflex contraction in the same muscle; the reflex twitches in this case are inhibited. In the same way it is established that general inhibition spreads both upwards and downwards from the place where, the stimulated sensory fibres enter.. and also in the other half of the spinal cord; the further it is spread, the weaker it becomes. The stronger the stimulation, the larger the area involved. General inhibition is chiefly induced by the spread- ing of the dorsal root impulses along the dorsal columns, but it is partly induced by impulses from the internuncial neurones. Inhibition occurs in both the motor and internuncial neurones. Numerous experimental facts aree presented to give weight to these conclusions. The anatomical substratum which produces excitation, and the substratum 'which produces inhibitionj, must be different, since (a) in- hibition arises before excitation add lasts longer, and (b) inhibition is alwaya general, but excitation spreads along definite paths. Besides the neurons paths - calls and their axona - along Aich the impulses of excitation are conducted in the -spinal cord, there is also neuropil - a net- work of nerve endings and dandrites - which brings about the inhibition of the grvo fibres and nerve calls. 1he neuropil must enter into an active state under e in uence of impulses passing along the dorsal-root fibres and the axons of -3- the internuncial neurones., in so far as they have synaptic connections with the dendritic net-work. The dendrites receiving these impulses are Laiable to conduct excitation (according to the author's conception), hence only "Local processes can arise. With these local processes weak electrical potentials appear. However, with the summatibn of the potentials of an innumerable quantity of dendrites, considerable slow potentiQla arise. Those neuropil potentials, spreacLing through the tissue fluid, have an olectrontonic action on the neighbortng cells and axons. The neuropil may be activated directly by applying to the dorsolateral surface of the spinal cord acetylcholine, nicotine or KC1 in very weak concentra- tion; in this case activation takes place only in the perimedullary and subpial layers of the neuropil, which can in its turn activate the neuropil of the central greynatter through the axon connections. To sum up., there is pure inhibition without excitation of the motor neurones. Under normal conditions such a pure form of inhibition is apparently obtained under the influence of afferent impulses- in those cases when the sensitivity of the neuropil to peripheral impulses for some reason or other is greater than the excitability of the cell bodies of the neuroneso Facilitation may be of different origin: (a) transitory increase of the excitability of the mot-or or internuncial neurones - as a result of preceding activity or under the influence of subliudnal impulse; kb) prolonged facilitation under the influence of the metabolites arising in connection with excitatioil (e.g. acetycholine); (c) general facilitation after general inhibition by the sudden cessation of the neuropil potentials (as in experiments with anelectrotonus on the peripheral nerie trunks)a Oedevani-lbillai SiOt Excerpta Medica, Section VIII Neurology and Psychiatry, Vol. 1.
