Operational ERB Meeting Report
1. The Operational Engineering Review Board was held on 17 December:
1987 at 1330. See Attachment A for a list of attendees. See Attachment B
for the Agenda.
2. Two new DR's were discussed. See Attachment"C-for a description
3. The O/BRB reviewed ail the In Progress DRs. See Attachment D for
of these DRs. Two walk-on DRs were also discussed and are listed in
a description of those DRs. The status updates from the ERB are also
`incorporated.
4. Those MRs scheduled for update were reviewed. See Attachment E
Attachment C.
for the current status of all MRs.
5. Two new RFCs were discussed. See Attachment F for a description
`discussed. See Attachment G for the current status of all RFCs. -
Attachment F. Those RFCs in process and scheduled for update were
of these RFCs. Two walk-on RFCs were also discussed and`are listed in

a. B. Pratt presented a Build Request, HU-BR -E-001-87, to
the O/ERB. This Build Request was presented to the 0/En for two
reasons. First, B. Pratt wanted to present a proposed solution to
complete DRs HU-DR -E-096-87, HU-DR -E-118-87, and HU-DR -E-197-87.
Secondly, B. Pratt wanted to propose that the 0/ERB begin using Build
Requests as a standardized means of documenting and approving the
installation of corrections to DRs and changes resulting from RFCs on
the AFS System.
b. The 0/ERB approved the proposed solution to complete DRs
HU-DR -E-096-87, HU-DR -E-118-87, and HU-DR -E-197-87. B. Hunter
requested that the proposed change to the Commo Operator SOP be
reviewed once more with the C/WSS to verify whether Commo Operator
messages should be sent to Slot 1 or Slot 2.
c. The 0/ERB discussed the proposal to begin using Build
Requests as a standardized means of documenting and approving the
installation of corrections to DRs and changes resulting from RFCs on
the AFS System. The purpose of the BR is to completely document the
corrections or changes to the AFS System, identify the DR(s) or
RFC(s) to be closed by the BR, provide red-lined documentation (if
any), provide change installation, verification, test, and backout
procedures, provide the results of pre-installation testing,
designate a change installation date approved by C/AFSB, and provide
for ERB Chairman approval of the BR package.
d. J. Visor noted that the AFS Forecast of Activities form
is currently being used incorrectly as a kind of Build Request, and
recommended that the use of BRs be implemented and that the AFS
Forecast of Activities form be used only as a tool for activity
scheduling. B. Hunter stated that he felt the ERB had not been
providing adequate feedback to the users on the correction of DRs and
the completion of RFCs. The identification of DRs and RFCs in the BR
and the identification of the BR in the DRs and RFCs will improve
feedback to the users by identifying the changes before they are
made, identifying when they will be made, and identifying which DRs
they are to correct.
e. The 0/ERB approved the implementation of Build Requests
to document the correction of DRs and the completion of RFCs. The
final format of the BR form will be developed by the 0/ERB as
experience dictates. The BR processing procedures also need to be
finalized by the 0/ERB, but initially BR processing procedures will
be as follows:

1) The O/ERB requires that the results of all approved
DRs or RFCs to the AFS System software, database, hardware,
facility, procedures or documentation baselines must be
documented on a Build Request.
2) BRs will be submitted to the O/ERB for each
resolved DR or RFC prior to implementation. A BR may contain the
documentation of more than one resolved DR or RFC.
3) The O/ERB will review BRs, verify priorities and
completeness, and authorize implementation of the results of all
approved DRs or RFCs to the AFS System.

Embedded TELEX messages did not get passed. and to

Analysis:
The TELEX driver did not recognize the EOM and SOM character set for the 2nd and
subsequent message on a receive of mail from TELEX.
Corrective Action:
Added code to trap for the different EOM/SOM character sets from RCA so messages
are passed correctly.

Problem Description:
Unexpected symbiont process termination. Insufficient virtual memory error.
Switched over from AFS1 to AFS2 and system ran fine.
Analysis:
DEC SPR 15.30.17 pertains. But symbionts do not correctly relaese virtual memory
as they run. This phenomenon is directly related to the number of print
requests the machine processes ad hoc.
Corrective Action:
This failure will usually only be observed after 8 to 10 days of continuous
operation and was masked by weekly switchovers. STOPFBIS/STARTFBIS should clear
the problem when it occurs.

Problem recurred on Dec. 14 at one work station only.
Corrective Action:
K. 'Woodard deleted directory, pubs sent files back through spooler, and report
was successfully built. 12-17-87: Bonnie, Kathy informed. They deleted file &
pubs started over.

Impact: If this can happen, pubs has to be constantly on alert to number of files
in a report. 12-17-87: This third recurrence in two working days. Holds
up report production. 12-17-87 No impact operationally.

When ATEX receives an incoming flash(Z) or immediate (0) msg, the batcher
broadcasts to SLOT1 that the msg is incoming. If SLOT1 has at the same time, a
msg pending or a system generated msg, the broadcast is overwritten and not seen
by SLOT1 editor.

FLASH precedence sorted alphanumerically. Changed Z to 1Z so it will be at top
of sort. Visual alert failed when terminal was busy w/other process. Visual
alert will follow completion of other process. Report on 11/19/87 by LEC for

Solution's on the system but need Xystrip tape problem solved before the
information can be entered into the queue as part of nightly maintenance
function. Workaround: Maybe distribute 410 pgs of laser proof per day or read
