I.n t_r o d u c.t i o n
In October 1960 9 years have elapsed since the- first
expedition of the Institute went to state agricultural farms,
state centres of machines and agricultural cooperatives of
Machn6w. The- aim of the expedition was to carry out some
investigations concerning the sanitary conditions under
which worked and lived rural workers and tractor-drivers..
The members of the expedition had also to examine the con-
dition under which worked those employed in application of
artificial fertilizers and dispersing of insecticides. The
actual state of the safety of labour at that time was very
unsatisfactory.
Tractor--drivers and labourers on farms worked under very
unsanitary conditions and nobody cared a bit to improve them
or make them less harmful for the health.
The origin of the Institute is closely connected with
Prof,Dr Witold Chodiko, the eminent hygienist and the first
Minister of Health. (He took post in 1918). During many
years he represented Poland in sanitary organisations of the
loague of Nations.
ItisWell known that the problem of care health and protec-
tion of labour is very large and requires more laboratories
and scientists to be worked upon satisfactorily. In view of
all this the Institute, following suggestions of some agri-
cultural institutions and surgeons acquainted with actual
needs of the countryside, included in its research plan du-
ring five years the following trends of scientific works:
General sanitation in the countryside.
Sanitary conditions in the countryside in.regard to
application of various machines and chemicals and rearing
of livestock in agriculture.

Anthropozoonoses and rural parasitology.
Preventive measures in controlling occupational rural
diseases especially traumatic injuries, skin diseases
and women diseases.
Studies on the conditions under which a disease is born
and the investigations on the absence due to diseases.
The section of occupational diseases requiring clinical
treatment has its resort of rehabilitation at Idalgcz6w. There
in our opinion all rural occupational diseases should be
treated.
The. research investigations of the Institute are carried
out in laboratories and under field conditions. These last
are carried out by special expeditions working collectively.
The expeditions are organized every year in the voievodships
of Lublin, L6d9, Opole, Poznan and their aim is to examine
the sanitary conditions of.workers employed in destroing
Colorado Beetle by applying great quantities of chemicals
injurious for man's health. Twice a year in spring and au-
tumn are organized expeditions to the State Centres of Ma-
chines. Their aim is to examine physiology of labour and sa-
fety of labour of tractor-drivers, oombain-drivers and wor-
kers, empoyed in threshing. The similar expeditions are orga-
nized to various villages of the District of Lublin to exami-
ne houseaccamodation conditions, personal cleanliness of
peasants, sanitation of rural building and the degree of
infestation of rural population with worms. Here must be
mentioned 6 camps organized by the Institute and the Medical
Academy for students and assistants in the villages. These
investigations carried out by a considerable quantity of
people at a time enabled to gather records which pictured
actual sanitary conditions and the healthiness of the exami-
ned villages. The greatest success of the Institute was to
control tularaemia in the woievodship of Szczecin. The expe-
dition sent for controlling it succeeded being assistedeffec-
tively by the State Office of Hygiene and the Institute of
Marine Medicine. Another success was to control swamp fever

in some villages situated on the Bug and in the vicinity of
Canal Ydieprz-Krzna. (1955-1960). It must be stressed that
the Institute gives.much time to research works under field
conditions completed by laboratory examinations. In order to
make these investigations scientific very often technical
equipement and needed apparatuses are transported to a vil-
lage where schools and buildings are occupied for arranging
field laboratories.
Since the very beginning of the existence the Institute
is collaborating with rural stations of health and Sanitary
Epidemiologic Stations in various voievodships. The Insti-
. tute has research workers in the whole country g who are in
contact with it and who are sending much information under
its address from all parts of the country. Among them are
chiefly rural surgeons. Thus the Institute has people colla-
borating with it in nearly all voievodships. One may call
them "scientific correspondents". The Institute counts 33 . of
them. Besides about 474 persons are collaborating with the
Institute permanently. Among them are eminent surgeons wor-
king in the countryside.
Every year (1954/1960) in the Institute are held sessions
of rural surgeons of the whole country. They have contributed
to the developement of rural health service. The Institute
has organised XIV courses for rural surgeons having as a pur-
pose raising their qualifications in the domaine of sanita-
tion.
The countries which develop modern agriculture are facing
some problems of vital importance such as increasing of crops
production, supply of greater quantity of meat and grain which
will help to raise the standart of living of the working
class, broad masses of the working peasantry and the inhabi-
tants of big towns. These problems are related with the three
factors. They are as. follows: ,
1. The developement and advance of agricultural science
and practice
2. The improvement in the organization of labour and in
agriculture economics, in the domaine of such socialist

arrangements as state agricultural farms, state machi-
ne centres and agricultural cooperatives.
3. The improvement and rapid progress in safeguarding
the health of rural population (rural occupational
medicine).
The builders of the modern agriculture should have con-
stantly in mind the importance of those three factors which
form an integral whole. They should take interest not only
in furthering planned farming and other collateral agricul-
tural problems. They should have in mind the benefit of man,
woman and young people engeged in collective farming and
raising the productivity. The conditions under which they
workI the problem of labour safety, the hygiene of their
accomodation and health should not be overlooked. Meanwhile
there are economists and farm managers who underestimate tho-
se social and medical problems and who neglected the-hygiene
and prophylaxis. Our duty is to explain those problems in
the press. Special importance must be laid on elucidating
the opinion: the labour in the countryside is healthy and
needs no assistance from the medicine. There is no healthy
work even in the countryside which can do without help of
social medicine and its preventive measures.
There exists special institutes of rural occupational me-
dicine and rural health which have to supervise the safety
of labour, the protection and hygiene of work. The industrial
occupational medicine may boast itself of having the expe-
rience, trained staff and equipement in this respect. I shall
enumerate here the injuries which endanger workers health.
Here they are grouped in six sections according to records
got by experience:
1. Plant production
2. Animal production
3. Wood production
4. Agrotechnical, zootechnical and wood industry,
5. General Hygiene and social conditions in the coun-
tryside

6. Work in agrotechnical and zootechnical stations, in
veterinary institutes, in professional and higher
schools.
The injuries related with plant production have their
source in:
a) Application of electric power to agriculture (vibra-
tions of machines), all sorts of poisons and fumes
attendant to machines, heat and cold, dust, climatic
and meteorological factors, all sort of injuries,
electric current, benzine, oil, skin diseases, taking
irregular and cold meals, seasonal bodily exertions,
night-work, sunheat, water for drinking, alcohol.
b) Using. chemicals in the countryside (in rural work).
artificial fertilisers
insecticides
fuels (benzine)
isotopes
dust.
o) Ploughing: the infections related with the work on
earth: (tetanus, gangrene, infections due to some
plants, actinomycosis), necessary, longday bodily po-
sition during the work sunheat, humidity,.-cold, windy
dust, pollen, allergic dermatoses and the like. Wor-
kers health is endangered by wyter works '(building of
canals, drainage works).
Harmful effects related with raising of livestock. They
concern rural workers, zootechnicians, veterinary surgeons
and other empoyees engaged in raising cattle, pigs, horses.
sheep, poultry. The harmful effects are caused by microorga-
nisms which are transmitted to men from affected animals by
contact with milk, mud, dust, urine. These microorganisms
include bacteria, fungi, yeast, protozoa. They may be trans-
mitted to men by animals, insects (ticks, flies) and worms.
The most important anthropozoonoses which occur in middle
Europe as occupational diseases-are: rabies, smallpox,
dairy-men pustules, foot and mouth disease, ornithosis, cats

claw disease, encephalitis, influsnzy, virus infection, ant-
hrax, tuberculosis, Blinders, erisipelas, salmonellose,
brucellose, pasteurellosis, tulareamia, leptospirose, li-
sterello.se, streptococcal infections. Infections due to some
bacteria, Q fever, toxoplasmosis, balantidiasis, trichino-
sis, taeniasis. Among raisers of livestock also popular
diseases such as allergoses (skin allergoses) asthma, inju-
ries caused by animals, dairymen rheumatism and others.
In forestry there may occur injuries caused by some cli-
matic and meteorological factors, wind, cold, humidity, fati-
gue, trauma, chemicals, irritations due to some plants and
insects. All of them affect skin, eyes, respiratory system.
Institutions dealing with care health are interested in the
following sections of industry:
A. Meet industry including sloughter-houses, shops selling
seasoned meat, factories of tinned meat and the like.
Milk and cheese manufacturing.
Tanning, Furwool, horse hair and feather manufacturing.
Products of bones. Manufacturing related with hunting
and fishing.
B. Pant industry. Flour and sugar manufacture. Spirit
and wine manufacture. Linen and cotton industry,
chemical industry (artificial fertilisers, insecti-
cides) Tobacco industry.
C. Wood industry.
Unlikely the workers'and miners'conditions'of living those
of rural workers and peasants are closely related with their
work. The question in point concerns chiefly the personal
cleanliness of the workers, men, women, and young people,
the hygiene of their hause accomodation (workers 'hotels),
the hygiene offood (the meals taken individually and in agri-
cultural cooperatives), the hygiene of water and of the
milieu (stables, cowsheds, pigsties). The sanitary problem
is also of great importance. It concerns sewage, water clo-
sets lice, fleas, mosquitos, rats, mice.

There may be also injuries due to the work in laboratories,
in agricultural, zootechnical and veterinary stations, in
institutes, professional and higher agrioulural schools.
In view of all this one may conclude that the problem of
hygiene and that of injuries endangering the workers health
involve broad masses of rural workers and of intelegentsia.
As they axe chief builders of socialist agriculture they
must be embraced by medical assistance scientific as well
as technical. The safeguarding of their health, the safety
of labour, prophylaxis must be included in the programme of
our Institute of Lublin. There is no need to stress how great
are the losses concerning work-day units on state agricul-
ture farms in agricultural cooperatives because of the absen-
ce of, workers caused by disease, diablement, mortality, pe-
riods of health recovery.
It is very useful that our Institute collaborates with
similar institutes abroad. Here must be mentioned Cheohoslo-
vakia,' Soviet Union, U.S.A. France, Bulgaria, Rumania, Greece,
England, Chile, Hungary, Germany and others, as well as the
World Health Organization.

Establishment of the State Institute of Rural Occupational
Medicine and Rural Hygiene
According to the 41-th ? I of the decree dated December
15-tb 1951 conoearning the high education,- the scientific
research and the scientists the Government Council decides
what follows:

1. To separate the Institute of Rural Occupational Medi-
cine from the Academy of Medicine in Lublin, working till
now as a part of the Academy and to form the scientific in-,
stitute called: "State Institute of Rural Occupational Medi-'
cine and Rural Hygiene".
2. The new-formed Institute is under the control of the
Minister of Health.

The Institute plans and carries out research into the
hygiene of work, preventive medicine and the treatment of
the professional diseases, caused by the farm-work, espe-
cially in the plant and animal production in the agricultu-
ral and forest industry.
The Institute is to be in Lublin.

The Institute is legalised.

Means to continue research of the Institute are assigned
by the Minister of . Health in agreement with the Minister of
Finance and the Governements economic commission.

1. The State Institute of Rural 'Occupational Medicine and
Rural Hygiene in this pages for abbrevation named the Insti-
tute simply, is active on the.ground of the bill nr 46/55 by
which the Council of Ministers, on the 15.1.1955, has set
up 'the State Institute of Rural Occupational Medicine and
Rural Hygiene in a detailed manner its activity is based on
the present Statute.
2. The Institute is located at the town Lublin.
3. The Institute depends on the Minister of Health.

1. The Institute is planning, organizing and earring on
scientific studies on the hygiene of work, its prophylactic
and the medicine of occupational diseases occurring in rela-
tion with rural labour, the studies are also concernad with
rural hygiene.
2. In particular to the tasks of the Institute belong the
following:
1) to elaborate the plan of scientific research works
dealing with labour welfare and the cure of occupatio-
nal diseases occuring in rural work, most of all those
related to plant and animal production,-agricultural
manufacturing and forestry, the studies include as
well general and communal rural hygiene,
2) to supervise and coordinate the scientific research
works belonging to the field of activity of the Insti-

Lute, these are carried out by scientific branches
which depend on particular Ministers and, in agreement
with the Minister of Health are entrusted with duties
by them,
3) to study biological and social problems concerned with
work in farming, agricultural manufacture and forestry,
4) to work out schemes and evaluate the yet existing
arrangements referring to diagnosis, eure and preven-
tive measures,
5) to cooperate in raising the professional qualifications
of,the physicians and of the medical auxiliary staff,
who deal with health preservation of farm and forestry
.workers t
6) to prepare the scientific staff,
7) in so far as it lies in the field of activity of the
Institute to give methodical supports to sections of
the Ministry of Health and other Ministries as well
as to scientific stations and social institutes of
the Health Service,
8) to collaborate with the Central Institute of Labour
Preservation for elaborating and estimating scientific
methodes well as arrandments in order to raise the
hygiene of labour in farming, agricultural manufacture
and f orestry,
9) to collaborate with appropriate institutions for the
professional reeducation of invalids whose disability
is due to labour in farming, agricultural industry and
forestry,
10) to cooperate with editorial institutes for publishing
the results of scientific researches,
11) to elaborate methods for carrying out health education
in the country,
12) to collaborate with national and foreign institutes
and organizations which activity comprises the same
problems as those of the Institute,

13) to express opinions and solve'
problems the Institute
is entrusted with by the Minister of Health. .

1o For organising and conducting its work the Institute
has its own means and a quipement, furthermore it gathers
the materials required for the realization of its plans
from medical academies and other scientific departments as
well as from social branches of the Health Service.
2. The Institute adresses to the Minister of Health its
proposal of entrusting other scientific departments being
subject to him as well as Medical Academies and branches of
the Health Service with research works related to the acti-
vity. of the Institute.
3. In the case if specific domains of the Institute acti-
vity belong to stations which do not depend on the Minister
of Health, the Institute conducts its work in agreement with
these stations.
4. The principles of collaboration with scientific sta-
tions, as determined by the bill 3, are established by the
Minister of Health and other Ministers (Board of the Polish
'Academy of Science) who are supervising these stations.

1. The Director is the head of the Institute, leads the
whole complex of works and is responsible for it.
2. For managing the scientific side the Director has the
help of a scientific secretary.
3. Both, the Director of the Institute and the secretary
are appointed by the Minister of Health who choses them from
among independent scientific workers.

1. To the tasks of the Director belong the following;
1) to lead the whole work of the Institute,

2) to elaborate projects of plans of studies,
3) to submit for sanction to the Minister of Health the
project of planned scientific works,
4) every three months to present to the Scientific Council
reports about the activity of the Institute taking
into account the performances of particular organi-
sative sections,.
5) to present to the Scientific Council all affairs which need
the opinion of the Council,
6) To submit for approbation to the Minister of Health
the bills of the Scientific Council dealing with the
following subjects:
a)
plans of scientific research works,
b)
reports
on the activity of the Institute,
c)
changes
require
of organisation in the Institute which
the approval of the Minister.
7) to initiate, if needed, new posts of scientific labour,
in the frame of the existing budget.

1. In the Institute is active the Scientific Council.
2. The organisation of the Scientific Council and the
detailed manner of its labour perfomance is determined by
a special regulation accepted by the Minister of Health.

1. The Scientific Council is composed of the following
members: the Director of the Institute as its leader, and
scientific workers as well as experts in problems which lie
in the field of activity of the Institute, the number of
these members can not surpass nine and no less than half of
them must be independent scientific workers.
2. Moreover, the folowing can take part in the meetings
of the Scientific Council:
1) leaders of sections and divisions of the Institute,
who are not members of the Council,

2) representives of the Minister of Health, agriculture,
State Agricultural Farmes., Forests, Agricultural In-

Polish Academy of Science and of the Central Institute
of Work Preservation,
3) representives of the Central Institute of Trade Union
and of other branches of social work which being in-
terested in the activity of the Institute are invited
by the Minister of He alth and delegated by their in-
stitutions,
4) experts in problems being the subject of the debate,
invited by the chairman of the Council,
5) substitutes for the Chairman of.the Council and for
the secretary summoned by the Minister of Health from
among the members of the- Council.

To the. field of activity of the Scientific Couoil belong
the following
1. Attention to the proper direction of the scientific
activity of the Institute.
2. Opinions concerning the scheme of the planned scien-
tific research work, as well as the estimation of the
fulfilment of the plan.
3. Initiation of scientific research works.
4. Expression of views with regard to the organisative
changes in the Institute.
5. Opinions .about the scheme annual estimate of revenues
and expediture of the Institute. .
6. Revision of rxperts mentioned in ? 5, point 4.?
7. Proposals addressed to the Director of the Institute
in connection with labour coordination and control in
particular organisative divisions of the Institute.

8. Revision of other questions, the council is either
entrusted with by the Minister of Health or they are
addressed by the Director of the Institute.

1. The Institute comprises the following organizative

of Anthropozoonoses,

Section ' of Work Hygiene of Motor Agriculture Workers,

Section of Rural and Sylvatic Toxicology,

Section of Administration and Economy.
2. For carrying on determined scientific investigations
can be summoned, with the agreement of the Minister of
Health, research groups, either durable or variable.
3. The Minister of Health, on the proposal of the Direc-
tor of the Institute, ratifies the detailed structure of the
organizative interior sections mentioned in bild 1.

. 1. For the benefit of persons who are not units of sociali-
zed economy the sale of any durable means of the Institute
is not allowed, neither is allowed to establish on them any
real limited rights.
2. In particular, economically well grounded cases the
Minister of Health in agreement with the Minister of Treasure
and the consentment of the Head of the State Committee of
Economic Planning can Jgive Institute permission for selling.
particular durable means or determined kinds of them to per-
sons, who are not units of socialized economy, or the founda-
tion on these means of real limited rights can be permitted.

1. For declarations made in the name of the Institute
referring to its financial obligations and rights the coope-
ration of two authorized persons is required.
2. Authorized persons are the following: the Director
and assigned mandataries. Mandataries deal in the limit of
their authorization.
The Minister of Health establishes, summons and recalls
mandataries and determines to what limits they are empowered.
He also can give the Director an authorization for indepen-
dent declarations in a special filed of action.

1. For performing functions of determined kind, or special
functions, mandataries working jointly or separately in the
