REQUESTS FOR TRAVEL AND/OR CONTACT
SECURITY
27 December 1966
1. Effective immediately all Requests for Travel and/or Contact
(Form 2504) should include travelers' social security number if
clearance certification is required.
2. Travelers should also remember to carry their social security
card with them for purposes of identification.
3. These instructions are in keeping with the Department of Defense
visitor control procedures now in effect at many of the installations
visited by NPIC employees. The NPIC Security Branch will forward the
social security number along with the visitor's clearances; the visitor
may expect to be challenged for his social security card.
4. The Security Branch has social security forms for those employees
who require duplicate social security cards.
