Smirnov, Zhurnal Mikrobiologii, Epidemiologii i Immuno- %. biologii, No 10, Oct 56, pp 86-9o "Analysis of incidence statistics for brucellosis among various groups shows that 75% of the yearly incidence of brucellosis (in the USSR] occurs among workers on animal husbandry farms, sovkhozes, and kolk-hozes, and in enterorises processing animal products .... Although the number of cases of biacellosis is comparatively low. the loss of work time due to incapac- itation by brucellosip on stock-breeding farms and in enterprises process- ing aninial products is very high. Thus, for example, in 1953 the loss of time from incapacitation due to brucellosis compared with the loss from all other diseases, was as follows on stock-breeding sovkhozes: in the Kazakh SSR, 37% in the Kirgiz SSR, 231t; in the Uzbek SSR, 19%; in Rostov- skaya Oblast, 16.8%; and in StalingradsKaya Oblast, 13%. In meat com- bines work lo-s attributable to the same cause accounted for 3-9% of the total number of days lost due to all diseases. These figures do not com- pletely reveal the serious damage which brucellosis does to the health of workers. The exhaustive nature of this disease, its refractory and pro- longed course, frequent exacerbations, relapses, serious complications, and the absence of a radical method of treatment often lead to partial or complete loss of' work capacity. Hardly any- other infectious diseasa of Occupational origin is the cause of suc'- & high percentage of temporary and permanent invalidism.-." It is then shown that "one of the main deficiencies in the work of the public health organs in controlling brucellosis is the fact that pro- phylactic vaccination and other preventive measures are instituted in an area only after the disease has broken out, and then only in places where cases have occurred." This is apparent since "the incidence of the dis- ease has decreased on farms where cases have occurred and increased on farms where no cases have been reoorted previously .... New brucellosis foci and flare-ups in old foci continue to occur in the Central Asian republics and in many oblasts of the RSFSR. It.must be taken into consid- eration that the organs of the veterinary service, for all practical pur- poses, are still in nc condition to guarantee the protection of disease- free farms against the introduction of infection or to immediately detect an incipient epizootic .... In regions considered enzootic for brucellosis, any stock-raisin-- farm can be considered a potential brucellosis focus." "According to the data of the Rostov, Saratov, Stavropolsk, Chk-alov, and other antibrucellosis stations, if vaccinations are carried out in the prescribed manner and at the right time, the incidence of brucellosis among persons vaccinated will be '5-6 times' lower than among unvaccinated persons. It would appear that a sharp reduction in the incidence of bru- cellosis among the population of these oblasts could be expected. However, this has not occurred; in fact, under ideal conditions, the actual reduc- Olt T, tion in incidence in any given oblast has not exceeded 50-6 . do basic factors i-make It difficult to obtain the effect which might be expected to follow the prophylactic vaccination of workers and officials on stock- raising farms: (a) the incomplete and untimely nature of the vaccination of permanent ', and especially returning and temporary workers, in active brucellosis foci, and (b) the unjustified prohibition of vaccination on so-called unthreatened animal husbandry establishments, where by decre2s and instructions issued by the Ministry of Health USSR prophylactic vac- cinations can be carried out only on individual farms, and then, strange to say, only after cases of brucel.Losis have occurred on them among workers or officials." "The present method of prophylactic vaccination consistiig of the subcutaneous injection of the dry live vaccine of the Institute imeni. Gamaleya suffers from a serious deficiency: the technique of admini6- tering it is extremely complicated and labor-consuming, making extensive use of this method difficult. In addition, there is a second deficiency -which reduces the epidemiological effectiveness of the subcutaneous method of vaccinationi about 65% of those vaccinated exhibit a positive allergic reaction at the time of their next regular revaccination and ar! therefore not revaccinated even though their vaccine immunity has already been par- tially or completely lost. Thus, these persons are unprotected against large doses of the pathogen during the extremely dangerous calving and lambing periods. An ~reactive cutaneous method of vaccination which does not have the aforementioned deficiencies has nroved to be completely re- liable and immunologically effective for humans, according to research carried out by N. F. Zenkova under the direction of Prof 1. K. Karakulov. Practicality dictates that a change-over to this method of vaccination be made even though its epidemiological effectiveness is no greater than that of the s-,focutaneous method. It is the immediate task of the scien- tific research institutes to put at the disposal of practicing physicians a reliable, simplified, areactive method of vaccination." "Prophylactic vaccination, which is the fundamental and foremost method used by medical workers in the arsenal of antibrucellosis measures, is, unfortunately, not a radical method of controlling this infection as are, for example, vaccinations against small pox and tularemia." The report Concludes with a discussion of insurance compensation to persons incapacitated by brucellosis and says that existing laws afford the trade unions a means of forcing the managers of stock-raising farms and meat combines to comply with instructions and directives concerning the control of brucellosis. SI-aRNOVI S. M "An Occupational Outbreak of Tularemia Among the Workers in a Meat Combine," by S. M.- Smirnov, Zhurnal Mikrobiologii, Epidemiologii i Immunobiologii., Vol 27, No 9, Sep 56, pp 37-43 An unusual outbreak of tularemia in October and November 1954 among vorkers of one of the meat-preserving combines in the Transcaucacus is described. A brief history of this outbreak, including clinical symptoms and initial diagnoses of malaria, influenza, and brucellosis" is given. Negative results from various tests and the presence of bubos character- istic for tularemia led to the final diagnosis, which was confirmed by isolation of cultures of the pathogen. After prophylactic vaccination (27 and 28 Oct) against tularemia, new cases of the disease subsided sharply among the workers and had disappeared by 9 November. Characteristics of the outbreak and clinical forms assumed by the disease in several cases are described. An epidemiological analysis of the etiology of outbreaks observed and data from experimental investiga- tions led to the following conclusions: 1. Under definite though not yet studied conditions among sheep, an acute epizootic of tularemia can arise; the sheep then become a dangerous source of mass infection of workers in meat combines and slaughterhouses. 2. Sheep infected with tularemia present a great danger during the period of generalized infection, which lasts from the second day after infection to the fifth or sixth day and proceeds without visible clinical manifestations. 3. The basic mechanism of transmitting infection in meat combines is contact with the blood or lymph of sheep suffering from tularemia dur- ing the period of bacteremia. 4. The probability of human infc:~tlion is diminished after removal of the blood from the sheep and especially after rigor mortis sets in. As a result of this., the degree of danger from commercial meat and by- products is sharply decreased. 5.
