the experiments was formulated (R-11 or NO 2) . As the stfirt-
ing material compound (I), R-H, the trans-4-stilbene, was
iised. The intermediate produot in this synthesiv sQheme, thL.
4-nitrotolane, was also previously unknown. As an inter-
esting synthetic peculiarity it was found that of bli the
subatancestried for the reduction, Ila 2S2 (NH4 )2 5, H2% in
1),~ridine solution, hydrazine hydrate in the prevence of a
nickel catalyst, and others, only that suggested by Ruggli
(!uggli, Ref 1), phenylhydrazine, was found to be a vali(I

A group of Soviet doctors and engineers under the directionof constructor W.
Gudov have designed an apparatus'for a mechanical performance of angiorrhaphy.
The suture is done by means of clamps of tantalum wire. The ends of the severed
vessel are attached with their intima outwards to bungs of the instrument and
then approximated by lever action. The suture is put in position in fractions of a
second. At the Sklifossovsky Institute in Moscow the angiorrhaphy was carried out
in 26 patients with fresh wounds of the aorta, aa.aKlUaris and femoralis and other
arteries. Twenty-five had a complete recovery. One patient with an injury to the
axillary artery developed infection and had to have amputation. By means of the
same apparatus anglorrhaphy was performed in 32 patients with traumatic aneur
sms. The results were good in 25 of them and fair in the remaining 5. An end-

to-end angiorrhaphy is possible with this apparatus even with a separation up to

Between 1928 and 1955, plristic substitution of tile oesoplingus was performed in
tile Sklifosovsky Institute in 696 patients; skin was used in 6 of these, and the
colon in 2; in all the other cases the small intestine was used. It was possible
to mobilize a section of the small intestine of sufficient length only in 7510 of
cases. In the remaining 2516 of cases failure resulted from -lie unfavourable dis-
position of vessels in the inesentery of the mobilized small intestine section. In
1950 a group of engineers and doctors constructed an apparatus for applying
vascular sutures to vessels ranging from 1.3 to 15 mm. in diameter. Thereby a
new method in plastic surgery was made possible. namely the provision of an
additional blood supply for some organ or other by vascular anastomosis. In
cases of oesophagoplasty anastomosis between the second iadial vessel of the
mobilized mesentery of the small intestine and the left intei nal mammary artery
proved to be the most feasible method. It was applied in I I cases of antethoracic
oesophagoplasty. Not a single patient died. There was one failure, when thrombos-
is of an anastomosis formed as a result of acute vessel sclerosis which ended in
necrosis of tile upper end of the intestine. On the basis of these experiences the
application of a vascular anastomosis between the internal mammary artery and
the mesenteric vessel of the mobilized intestine seems undoubtedly to be a very
rational method in cases when it is necessary to furnish additional blood to the
intestine. In 10 cases of reconstructive oesophagoplastic operations, using small
intestine, anastomosis between the metienteric vessel and the right gastroepiploic
artery was applied when the blood supply of the mobilized intestine proved insuffin-
ient. The normal blood supply of the intestine was restored in every case. The
author also cites a case. in which additional blood supply of the mobilized intostine
was effected by connecting its artery with the middle colic artery.

Experim-e-n-fal data concerning end-to-end anastomoisis of ureters by means of an
apparatus for circular stitching used for repair of vessels are discussed. Eight
ureters were repaired in this way. Flow of urine was free on the second post-
operative day. The edgos of the anastomosed ends were apposed correctly without
any narrowing of the lumen. Tantalum clips entered only the outer layers of the
ureters and did not penetrate into the lumen. (S)

The ie-r-ti-o-n-aT-e-x-pe-r-re-n-c-e-o-t the author leads him to the conclusion that volon plasty
for an artificial oesophagus may be carried out by various methods which depend
oR the individual case. It in contended that the best colon region to ehoose for the
oesophageal replacement to the ascending colon and the right side of the trans-
verse colon with isoperistalsis. The mobilization of colon for the substitution of
oesophagus may yield a better result than that of small intestine, thanks to the
more favourable blood supply of the colon, its greater lumen (which makes the
junction with the oesophagus and the stomach easier) and absence of numerous
coils (the passage of food being thus proinoted).
