Please inform all employees at your station or base who are enrolled in the WAEPA life insurance program that additional insurance for eligible dependents (i.e., spouse and unmarried dependent children between the ages of 2 weeks and 19 years) has been added to the con- tract in accordance with the schedule outlined below: New Amount of Coverage Spouse $2,000 Children: 2 weeks and under 6 months 200 6 months and under 2 years )+00 2 years and under 3 years 800 3 years and under 4 years 1,200 4 years and under 5 years 1,600 5 years and under 19 years 2,000 Full-time students who are 19 years 2,000 but under 21 years 2. This new coverage is double the amount previously available under the contract at the premium rate of $.30 per month. The cost for this additional unit of dependent coverage is $.30 per month. Thus, the total cost for this higher amount of dependent coverage will be $.60 per month. 3. This additional unit of dependent coverage was effective 21 August 1966. The WAEPA Life Insurance Association has assumed the premium cost for the additional unit of coverage for the first two months (September and October). In the case of employees who already have the dependent coverage and whose premiums are being paid by pay- roll deduction, the cost of the additional coverage for November and subsequent months will be collected by payroll deduction, commencing with the pay period which begins 28 August 1966 since premiums are CROSS REFERENCE TO DISPATCH SYMBOL AND NUMBER DATE BOOK DISPATCH 25X1 A GROUP I CLASSIFICATION $QB 9ILE NUMBER E>
