25X1A 1. Attached USAF Accident/Incident Report pertaining
Heliocopter, HH-438, Serial Number 58-1847, has
been reviewed by this office.
2. All available information supports statements by
the Investigating Officer that maintenance was not a
contributory factor to the accident. It is also noted
that material failures were a result of, rather than a
cause of the accident.
3. The corrective actions appear satisfactory.
Recommend subject report be accepted and administratively
