SUBJECT: Source Evaluation Board for Civil Space Remote Sensing
Disclosure of Information
As a U.S. Government employee, I, the undersigned,
understand that during the course of evaluating contractor
proposals, shall not reveal any information concerning the
- evaluation to anyone who is not also participating in the same
evaluation proceedings and then only to the extent that such
information is required in connection with such proceedings.
Further, I understand that the right to information on this need-
to-know basis does not normally extend to the chain of
supervision of Board members or any other participants. I
certify that I will not disclose such information except as
provided herein, and that I will take adequate precautions to
assure against disclosure concerning any data and/or documents
which may come into my custody during the evaluation period.
Conflict of Interest
I have received a copy of the SEB Guidelines and my parent
agency's conflict of interest regulations, and have duly filed
(or will so file prior to participating-in subject proceedings) a
current Financial Disclosure Report (SF 278) or confidential
Statement of Employment and Financial Interest. I certify that I
have or will comply with all provisions of the above referenced
procedures and that I will promptly advise the appointing
official, through the chairman of this SEB, if I now or at any
time during the evaluation have any definable conflict of
interest concerning this procurement action.
