1. I hereby authorize the Pay Roll Section to deduct the sum of
50.00 per pay period from my salary being paid at
Headquarters, beginning with pay period ending 13 May 1961
2. (CHECK APPLICABLE STATEMENT(S).)

terest and will continue until my loan and interest thereon
is paid in full.

NOTE: At present the sum of $50.00 is being deducted and placed
in Share Account. This deduction should be continued. The
additional sum of $50.00 should be deducted and applied
against loan. Therefore the total sum of $100.00 should
be deducted per Four (4) week pay period.
This deduction will be credited to my share account and shall
continue until terminated by me in writing.
