Enclosed are the two claim forms which we discussed yesterday.
For your convenience, we have filled in some of the information but
please review the forms carefully.
Form 3104 is to be submitted in claiming a survivor annuity for
an The form should be completed and signed by you and
returned to me at the above address. Please note that a certified copy of
the death certificate should accompany the form.
There are also enclosed three claim forms (FE-6) for payment of
life insurance benefits. an should each complete one.
Should you decide to file .or nil as guardian, you should
complete the other form and forward it to me a ong with a certified copy
of the court appointment naming you guardian. We will also need another
copy of the death certificate to submit with the insurance claims.
