1. Mr. Harvey came to see me on 30 October 1967.
2. He was provided with the following:
a. Retirement application forms and relevant
instructions.
b. Hospitalization claim processing information.
c. Two hospitalization ID cards.
d. General information regarding his several
insurance policies.
3. Mr. Harvey has not as yet set his retirement date --
he has one more medical appointment. I believe his retirement
date will be 30 November. He may, however, put it off depending
upon what his doctor says.
4. Colonel White wants to_have a final interview after retire-
ment date is firm.
